Curious about J! paperwork

Recently I got curious about something: Does anyone have a copy (without your personal information entered of course) of the questionnaire/contract/whatever the paperwork is called that you have to sign before appearing? If so and it wouldn't be too much effort to do so, could you PM me it as a file? (If you've been sworn by TPTB not to show even a blank one to the public, please don't break the rules in that case.) I just thought about it and am curious as to what's on it. Thanks, Kelly
