As the head arbiter, I would rule against titles, honorifics, etc. being part of the name. Seems to be one step farther away from the original intent of First+Last = Title. (Here's a film with more than one: Captain Newman, M.D.)

I'll go through the rest of the offerings some time today and update the running list. I'm not surprised there are a lot more answers here than I might have originally thought. Keep them coming.
